Coordinates resource management for the call center. Ensures efficient screening, evaluation and triage of incoming referrals.
This is a hybrid position with in-office and remote work. Candidates applying must reside within 60 miles of Jacksonville, FL.
Responsibilities:
- Coordinates resource management for the call center, identifies and develops relationships with regional healthcare providers to establish support services that are realistic, appropriate and accessible.
- Creates and maintains provider database and ensures department staff are aware of current available resources. Addresses potential shifts in service care needs or health care trends impacting future resource management.

Experience & Qualifications:
High School Diploma or equivalent required.
1-3 years of relevant experience, required.
Bilingual highly preferred.
